In connection with Your application to participate as a service professional (“Provider”) on the BOOROCK LLC marketplace platform, and to evaluate Your ongoing eligibility as a Provider on the Platform, BOOROCK LLC (“BOOROCK”) may obtain a consumer report about You from a third-party consumer reporting agency.
The consumer report may contain information about Your: (a) criminal history, including records of arrests, indictments, and convictions; (b) sex offender registry status; (c) identity verification and fraud indicators; and (d) such other information as may be included in the consumer reporting agency’s standard background investigation report.

BOOROCK does not currently conduct background checks on all Providers as a mandatory condition of initial platform onboarding. However, BOOROCK reserves the right to require a background check at any time, including as a condition of continued participation on the Platform, and You authorize such checks as described below. BOOROCK will notify You if and when a background check is being initiated.
A Summary of Your Rights Under the Fair Credit Reporting Act is attached to this Disclosure, as required by federal law. Please read it carefully. It describes Your right to obtain a free copy of Your consumer report from the consumer reporting agency that prepared it, and Your right to dispute inaccurate or incomplete information. If adverse action is taken based in whole or in part on information contained in a consumer report, You will receive a copy of the consumer report and a description of Your rights before the adverse action is made final.
By signing below, I acknowledge that I have read and understand the above Disclosure. I authorize BOOROCK LLC, and any consumer reporting agency it may engage, to obtain one or more consumer reports about me, now or in the future, as described above.
I understand and acknowledge that:
(a) This authorization applies to any consumer report obtained during my application to participate as a Provider on the BOOROCK Platform and during any period of my participation on the Platform.
(b) A photocopy or electronic copy of this Authorization shall be as valid as the original.
(c) This Disclosure and Authorization is a standalone document and is not combined with any waiver of rights or any other agreement. My signing this form does not waive any rights I have under the FCRA or applicable law.
(d) The Summary of Your Rights Under the Fair Credit Reporting Act has been provided to me with this Disclosure.
